Authorization To Release Remains
This document needs to be completed, signed, and faxed to Crosby-N. Gray & Co. before we can transport the decedent to our facility.
Arrangement Preferences
This document is useful for preplanning funeral arrangments. You may bring the completed form with you when you meet your Crosby-N. Gray & Co. funeral director, or you can fax, email, or mail the form to your funeral director. All information will be held in strict confidence.
